Duties

Summary

The Cincinnati, OH VA Medical Center is recruiting to fill a few vacant Licensed Practical Nurse (LPN) positions located in the Primary Care Department.

Work Schedule: M-F, day shift, however you must be able to adjust work schedule to accommodate patient/program needs.

Responsibilities

This LPN is a member of a Patient Aligned Care Team (PACT) and works under the direction of a Registered Nurse (RN) or Primary Care Provider (PCP) and reports to the nurse manager assigned to his/her area of work. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN requires a general review of their work by a RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with a Teamlet (Patient, PCP, RN Care Manager and Medical Support Assistant) and Larger Team (clinic, family/caregiver, internal and community-based services and disciplines involved in providing care to the patient).
